Authorizing Someone to Act on Your Behalf

  • If you want to allow someone else to call [Custom.Number] and access your personal Medicare information:
    ⦁ Complete the "Medicare Authorization to Disclose Personal Health Information" form.
    ⦁ Find the form online at ⦁ www.cms.gov/cms10106-authorization-disclose-personal-health-information.
    ⦁ Or request it by calling 1-800-MEDICARE.

    Tip: It’s a good idea to set up this authorization in advance, in case you are unable to do it yourself later.

You remain in control — you authorize each app individually, and you can manage or revoke access anytime through your MyMedicare.gov account.

  • What You Can Do with Medicare Connected Apps:
    ⦁ Download and save a file containing your personal health information, including Parts A, B, and D claims.
    ⦁ Print or email your information after saving it.

    ⦁ Import your saved file into other personal health management tools.

    ⦁ Access and review your past and current Medicare claims, helping you make better-informed healthcare decisions.

  • Important Note for Medicare Advantage Plan Members
    ⦁ If you have a Medicare Advantage Plan, only your Part D prescription drug information is available through Medicare Connected Apps.
    ⦁ Contact your Medicare Advantage plan provider directly to check if they offer a similar service for accessing your claims.
